Remote - Licensed Practical Nurse - LPN - LVN

CareHarmony is seeking a motivated Licensed Practical Nurse (LPN) to provide value-based care management for patients. The role involves identifying and coordinating community resources, educating patients, managing medication, and assisting with clinical and non-clinical care. This is a full-time, remote position with a rotational on-call schedule.

Requirements

• Manage patient census with a resolution-driven approach.
• Identify and coordinate community resources.
• Provide patient education and health literacy.
• Perform medication management, including identification and reconciliation.
• Assist with timely service delivery (Home Health, DME, etc.).
• Resolve patient questions and manage referrals/appointments.
