Job Description
Care Dimensions has a truly meaningful purpose – to provide compassionate care to our patients who are faced with an advanced or end-of-life illness. As a RN Case Manager, you’ll coordinate all aspects of patient care, ensuring individualized, compassionate support that maximizes quality of life.
Requirements
- Active Massachusetts RN license in good standing
- Associate’s degree in Nursing required; BSN or equivalent preferred
- Proven ability to provide care with compassion and cultural sensitivity in diverse home or facility settings
- Strong interpersonal skills and the ability to thrive in a collaborative team environment
- Flexibility, self-direction, and sound clinical judgment
- Valid driver’s license and access to an insured, reliable vehicle
