Requirements
- DDS or DMD degree from an accredited dental school
- Valid dental license to practice in the relevant jurisdiction
- Current Arizona State Licensure Renewal Certificate
- Current DEA Number certificate
- CPR certified (BLS) & Valid Driver’s License
- Ability to read, write and speak English
- Good interpersonal and human relations skills
