Job Description
Adoration Home Health and Hospice is seeking a passionate and dedicated Home Health Social Worker for their Cordova, TN location. This PRN role provides expert, patient-centered care and offers a supportive, fulfilling work environment with flexible scheduling. The position focuses on providing 1:1 visits, assisting clients in obtaining funds, and working on individual or group conferences to improve their social functioning.
Requirements
- Provide 1:1 visits to make a lasting impact on patients and families
- Assist clients in obtaining government funds, education, or treatment
- Guiding and counseling individuals and families in crisis
- Assist clients in modifying attitudes and patterns of behavior
- Develop a service plan to restore social and health adjustment
