BHcare is seeking a Community Support Specialist to join our team. In this role, you’ll help individuals build independence, improve their quality of life, and achieve personal goals through skill building, recovery support, and community connections.
Requirements
- Provide individual and group sessions, supporting clients with daily living skills, recovery tools, self-advocacy, and health and wellness.
- Conduct functional assessments and develop personalized, stage-wise recovery plans that are reviewed and updated regularly.
- Monitor and document client progress while maintaining accurate clinical records.
- Offer crisis intervention and outreach as needed.
- Assist clients in connecting to medical, psychiatric, educational, vocational, and community resources.
- Provide education and support to families and natural supports to help clients succeed.
- Collaborate with the program team to ensure high-quality, person-centered care.
- Provide limited transportation as clients learn independent skills (use of personal vehicle required).
- Strong interpersonal and motivational skills.
- Ability to provide compassionate, client-centered care.
- Experience with psychiatric rehabilitation, recovery planning, or related services preferred.
- Flexibility to support clients in community and home settings.
- Strong organizational and documentation skills.
