Job Description
The Case Manager provides direct and indirect patient care to active-duty patients receiving inpatient behavioral health treatment, coordinating discharge plans and facilitating multidisciplinary collaboration. They are responsible for patient care goals, case management, and ensuring seamless transitions to appropriate levels of care.
Requirements
- Collaborate with clinical team to address patient care concerns.
- Develop and implement individualized discharge plans.
- Coordinate follow-up appointments and referrals.
- Identify and connect patients with community resources.
- Document patient interactions and treatment plans.
- Maintain legal compliance (e.g., 5150, 5250, 5270).
