Job Description
As a Visiting Nursing Services (Homecare) Intern, you will interact with the Atlantic Visiting Nurse team, see patients in their home environment, and help them achieve their health goals. You will work with specialty programs, including Remote Patient Monitoring and Virtual Care, to reduce the risk of re-hospitalization and provide elevated assessment and collaboration. You will also collaborate with an interdisciplinary team to report changes in patient condition and progress towards goals.
Requirements
- Interact with Atlantic Visiting Nurse team members in an office and community setting
- Work with visiting nurses to see patients in their home environment and help them towards their individual health goals
- Work with specialty programs, including Remote Patient Monitoring and Virtual Care
- Collaborate with an interdisciplinary team to report changes in patient condition and progress towards goals
