Job Description
We are seeking a RN Referral Specialist to work closely with hospitals and other facilities to coordinate continued patient care with hospice home healthcare services after discharge. The ideal candidate will have excellent customer service skills and be able to communicate effectively with patients, families, and referral sources.
Requirements
- Develops initial plan of care for a Hospice admission to Home Hospice, or to a General Inpatient bed
- Coordinates with patients, families, physicians, and discharge planning personnel
- Maintains current knowledge of clinical issues, regulatory and reimbursement standards, insurance contracts and negotiated rate amounts
