CAFS Case Manager

This position provides supportive mental health services to children and adolescents. The Case Manager provides referral to appropriate services, coordinates services between the client, client support system, community, and other healthcare and social service professionals, and acts as an advocate for the client.

Requirements

• High school diploma or high school equivalency diploma
• 18 credit hours of post-high school education in a field related to behavioral health
• 2 years of full-time behavioral health work experience
• Associates/Bachelor's degree in mental health/human services or related field
• Excellent written and oral communication skills
• Ability to speak the Apache language
• Strong critical thinking, problem-solving and organizational and time management skills
• MS Office proficiency
• Valid Arizona Driver's License and reliable transportation
