Requirements
- Minimum one year clinical and/or working experience in geriatric, rehabilitation, or long-term Care nursing.
- Some leadership knowledge/experience is considered an asset.
- Certifications obtained or in progress that include gerontology, mental health, behavioral support, and palliative care are an asset.
- Current registration with the College of Nurses of Ontario as a Registered Nurse in the Extended Class (Primary Health Care or Adult) and entitled to practice.

- Assess, diagnose, and evaluate acute and chronic illness and other changes in resident’s conditions, prescribe treatments, diagnostics and follow up care.
- Support and advocate for residents’ best interests while engaging with the resident / family / substitute decision maker (SDM) regarding Care.
- Participate in resident care conferences.
- Provide clinical leadership to the LTC team members centered around the resident care needs which can include resident move in/out, hospital discharge planning, palliative and end-of-life care, pain management, dementia care and responsive behavior management.
- Participate in identifying, analyzing, and interpreting trends in resident care and professional nursing practice to determine priorities for educational programming for team members, residents, and families.
