Nurse Practitioner - Loudoun County, Virginia

Advantmed is seeking a highly skilled Nurse Practitioner to provide in-home wellness risk adjustment assessments for Medicare members. The role involves conducting assessments on a population with chronic conditions, delivering patient health education, and building trust with members. This is a 1099 (PRN) position offering flexibility and the opportunity to be part of a growing provider network.

Requirements

• Valid unencumbered NP License for the state you will be working in
• Minimum 30 hours per month commitment
• Preference for weekday or standard work schedules
• 1099, PRN, part-time experience preferred
• Prior experience in in-home risk assessment is preferred
• Reliable transportation for travel
• Strong EMR system skills
• Ability to work independently
• Bilingual is a plus
