Advantmed is seeking a highly skilled and compassionate Nurse Practitioner to conduct in-home wellness risk adjustment assessments for Medicare members. This 1099, PRN, part-time position offers flexibility and opportunity to provide individualized quality care to elderly populations. The role involves assessing overall health and well-being through various assessments and providing patient health education.

Requirements

• Valid unencumbered NP License
• Willingness to travel up to 55 miles
• 3 years patient care experience (primary care, adult, or geriatrics) preferred
• Electronic Medical Record (EMR) experience preferred
• Ability to travel within a designated area
• Strong Bicultural communication skills
• Reliable transportation
