Job Description
Advantmed is seeking a highly skilled and compassionate Nurse Practitioner to join their provider network. The NP will be responsible for in-home wellness risk adjustment assessments for Medicare members in Hamilton County, TN, focusing on accurate risk adjustments and value-based care. This is a 1099 position offering flexibility and opportunity for those balancing competing priorities.
Requirements
- Valid unencumbered NP License for the state you'll be working in
- Minimum 30 hours per month of work
- Experience with in-home risk assessment (preferred)
- 2+ years patient care experience (primary/adult/geriatric preferred)
- Preference for weekday/standard schedules
- Travel up to 55 miles
- Strong ability to work with EMR systems
- Ability to work independently
