Job Description
The Care Manager provides services within the Care Management programs, overseeing and coordinating access to services for people with intellectual and developmental disabilities, developing and implementing person-centered Life Plans, and advocating for members to access needed services.
Requirements
- Bachelor of Arts or Science degree with two years of relevant experience
- License as a Registered Nurse with two years of relevant experience
- Master's degree with one year of relevant experience
- Fluent understanding of community cultural traditions, norms, and practices of Members and their families
- Excellent interpersonal, public speaking, and written communication skills
- Ability to work autonomously
- Demonstrate professionalism, respect, and ability to work in a team environment
