Job Description
ADAPT Community Network is seeking a Residence Program Specialist to provide care, development, and independence to individuals living in their residential facilities. This role focuses on dignity, respect, and providing support, skill-building, and advocacy to ensure residents achieve their personal goals and live fulfilling lives. The position involves direct patient care, collaboration with a multidisciplinary team, and adherence to best practices for resident well-being.
Requirements
- Provides care and supports the people we support.
- Ensures the safety, health, and well-being of the people we support.
- Teaches and assists with daily living skills.
- Documents activities, progress notes, and incident reports.
- Participates in interdisciplinary treatment plans.
- Maintains confidentiality and follows agency policy.
