Case Manager - Hennepin County

We are looking for Case Managers to work with people with disabilities residing in Hennepin County. The successful candidate will be accountable for the full individual's care cycle, assessing needs, evaluating treatment options, creating care plans, coordinating care, and monitoring progress.

Requirements

• Bachelors Degree in social work, nursing, psychology, sociology or a closely related field from an accredited four-year college or university
• Good written and oral communication skills
• Excellent at working independently and in crisis situations
• Comfortable with high-volume paperwork
• Attention to detail and process oriented
• Able to pass a DHS (Department of Health Services) background check, valid driver’s license, and clean driving record
• Reliable transportation
