Job Description
The Community Outreach Clinician supports individuals experiencing homelessness transitioning to stable housing. This role combines outreach, counseling, advocacy, and care coordination, fostering dignity and support through person-centered, trauma-informed care. The position requires flexibility and a collaborative, interdisciplinary team environment.
Requirements
- Licensed Master’s degree (LMSW, LMHC, LMFT, LCSW, etc.) or RN with experience in behavioral health and/or substance use support
- Passion for helping individuals experiencing homelessness or housing instability
- Comfort working in community-based, field-oriented settings
- Strong interpersonal skills
