Requirements

• Works closely with the interdisciplinary care team.
• Provides input to providers/patient/family for written individualized care plans.
• Reviews patient intake assessments and coordinates care plan completion.
• Assesses medication compliance and monitors adherence.
• Ensures patients and care givers are aware of test results.
• Organizes and participates in case conferences.
• Reviews benefits and assists with application.
