Job Description
Sun River Health provides comprehensive primary, preventative, and behavioral health services to underserved and vulnerable populations in Suffolk, Rockland, Orange, Dutchess, Ulster, Sullivan, Columbia and Westchester County. They are seeking a Full Time Children’s Care Manager to provide support to clients with a high need for clinical and support services. The role focuses on assessment, care plan development, case coordination, monitoring progress, and providing linkage to referrals.
Requirements
- Utilizes approved Children’s Health Home clinical tools and technology
- Develops, coordinates and integrates a coordinated care plan
- Tracks/ monitors client progress and produces/maintains case notes
- Provides linkage, coordination, referral, and follow-up
- Attends planning meetings with service providers
