Social Worker - LCSW

Requirements

• Maintain a clinical caseload of patients
• Participate in daily rounds
• Complete psychosocial assessments on patients
• Develop treatment and discharge plans
• Provide crisis intervention and counseling
• Develop treatment strategies to engage patients
• Follow-up and outreach to coordinate service delivery
• Manage and document all services
• Develop and implement intervention strategies
• Report any incidents to the appropriate authorities
• Evaluate and develop interventions for substance abuse
• Ensure confidentiality of patient information
