RN Care Coordinator, SOLAR

The RN Care Coordinator will lead the team of LVNs in the Recuperative Care program, providing on-site medical management and health education to participants, and liaising with community and hospital partners.

Requirements

• Experience as a RN, two years' preferred
• Knowledge of and practice in the methods of harm reduction
• Experience working with individuals with mental health and substance use issues
• Current valid California Driver's License
• RN Credentials
