
Job description
The Case Manager works as part of the interdisciplinary Integrated Mobile Health Team – Full Service Partnership (FSP) team to conduct assertive outreach to chronically homeless individuals and provide them with intensive services to link them to housing and resources. The Case Manager will assist clients with benefits and housing application processes, life skills, and other skills as needed, and provide crisis management, case management, and other therapeutic services related to substance abuse and mental health.
