Community Health Worker - Behavioral Health job description: Reema Health is seeking an experienced and self-motivated Community Guide to work with individuals living with serious mental illness and substance use disorder. The Community Guide will build genuine, positive relationships with members and improve their health and wellbeing through connection to health care and community resources.

Requirements

• 3 or more years of direct experience with population or community to be served and knowledge of community
• Bachelor’s degree in Social Work, Human Services, Psychology, Health Sciences or other related fields
• Flexibility to work independently and collaboratively in a fast-paced startup environment
• Strong propensity towards building and maintaining relationships through initiating conversations and creating connections
