Community Guide - Behavioral Health

Reema Health is seeking a Community Guide to support individuals with mental illness and substance use disorder in a community setting. The Community Guide will build relationships with members, coordinate care plans, and provide support through technology to improve their health and wellbeing. This role offers opportunities for independent work and collaboration, emphasizing a blend of direct engagement with members and supporting a team of clinicians.

Requirements

• Bachelor’s degree in Social Work, Human Services, Psychology, Health Sciences or related field (or equivalent experience)
• Fluency in Spanish
• 3+ years of experience working with a population of individuals with mental and chemical health needs
• Driving license and valid vehicle insurance
• Must be able to work flexibly and reliably with a valid driver's license.
• Proficiency with Microsoft Word, Outlook, and Excel, and Video Conferencing
