Case Manager (HFA Bilingual Advocate)

Maternity Care Coalition is seeking a Case Manager (HFA Bilingual Advocate) to provide in-person, strength-based support to expectant parents and caregivers through the Healthy Families America (HFA) model. The role requires bilingual fluency and the ability to work independently, with a focus on flexibility, meaningful conversations, and meeting people where they are.

Requirements

• Associates, Bachelor's, or master's degree in social work, Public Health, or related field.
• Bilingual fluency in English + Spanish or Haitian Creole, African Dialects, Indian Dialects, and French (speak, read, and write).
• Ability to work in-person and travel throughout Berks, Bucks, Delaware, Lehigh, Montgomery, and Philadelphia counties.
• Willingness to work non-traditional hours, including evenings and weekends as needed.
• Valid U.S. driver’s license and clean driving record.
• Comfortable working independently
