The Service Navigator, Addictions is responsible for providing navigation support to clients with substance use issues in the Sunnybrook Emergency Department or an inpatient unit. The person in this role will work closely with the Peer Service Navigator to support patients to access services they need to address challenges with their substance use and to improve their quality of life.
Requirements
- Assists clients in the development and implementation of a care plan tailored to the client's needs and issues designed to improve quality of life.
- Act as a liaison and client advocate among those involved with the client's care including family, other caregivers, medical and other multi-disciplinary providers.
- Provide supportive counselling or referral and linkages to health care, programs or service providers.
- Provide crisis management/intervention and assist in the development of crisis plans for clients;
- Develop respectful and trusting relationships with clients using Recovery and Psychosocial Rehabilitation principles;
- Maintain strict confidentiality and follow all other required policies and procedures;
- Consistently interact with clients, staff, family members, other community agencies and partners in a professional manner that reflects favorably on the organization.
- Attend and actively participate in all required training;
- Liaise, collaborate and coordinate with other professionals, service providers and partners;
- Participates in partnership meetings as required
- Participates in weekly team meetings, individual supervision and regular agency administration meetings
- Takes responsibility for identifying areas for professional growth and training, in conjunction with the Manager or Director
- Participates in ad-hoc committees within the agency, providing input to improve the services provided by the agency
- Represents the Network at external workshops, inter-agency initiatives, staff training and other community-based activities
- Complete other duties, from time to time, as assigned by Program Director/Manager
