HART Support Worker

We are looking for a dedicated, flexible, reliable, and enthusiastic care professional to join our Home Care Assessment and Reablement Team (HART) as a HART Support Worker. The successful candidate will provide short, intensive reablement support to people with disabilities and those who are frail and recovering from illness or injury, focusing on helping them to restore their optimum level of independence.

Requirements

• Hold a Level 2/3 Diploma in Health and Social Care (or other relevant equivalent qualification) or an equivalent level of relevant experience and a willingness to undertake the diploma/care certificate after induction.
• Be committed to delivering high standards of care/support.
• Be able to work effectively on your own as well as part of a team.
• Have basic IT skills sufficient to write and maintain records.
• Be able to communicate effectively both verbally and in writing and record relevant and accurate observations.
• Be able to work under pressure and to remain calm in crisis situations.
• Be willing to attend relevant job-related training as required.
