
Job description
The Health Worker 2 performs a wide variety of paraprofessional duties in a service program of the Department of Public Health. They serve as a liaison between community residents and program staff, provide counseling and advice to patients, and may supervise Health Worker 1. They may also drive or accompany patients between their homes, hospitals, or other social agencies.
The Health Worker 2 will participate in the health service training program, provide information and resources to patients, assist patients in utilizing health care services, and make follow-up contacts when required. They will also serve as a liaison between the professional staff and the community, provide language interpretation services, and assist in gathering and evaluating data concerning the program.
This role requires a combination of experience and skills, including serving as a liaison between targeted communities and healthcare agencies, providing culturally appropriate health education/information and outreach, and providing referral and follow-up services. The ideal candidate will have strong communication and interpersonal skills, be able to work in a fast-paced environment, and be able to maintain accurate records and files.
